If an Incursion ends up inside a Major Arkfall zone it will glitch out both the Incursion and the Major Arkfall. Making both of them unable to complete.

This happened on 360 as well.
I have shared this with the team, as I have seen it reported a few times. As you see this happening, please report it in game using our in game bug report feature.
The more bug reports we get, the more information we will have to work with.
