Near the start where you have to eliminate the Mutant Swarm, for me at least. After I killed the dual wieldin' looking mutant (Have not played Defiance in forever btw, so forgive me) I had a problem where the tutorial did not continue after I killed everything there.
But there was also a constant damaging field in said area where I had to constantly revive myself. I resolved the issue by logging out and joining back in.
