Just done the whole co-op mission on without dying and it did not register the bit where it say complete the final event without dying that all I needed to do to complete this goal has this happened to anyone else or have I done something wrong

Keep running it. It glitches out sometimes and counts for people who did go down versus people who did not.
it's happened to me too and didnt give me credit for shutting down all the generators. even the other coop maps like explosions 101 and soleptor excavations' quests are bugged. it took 5 playthroughs for the kill the hulker in under 3 minutes to pop up. i hope it doesnt take that many for this particular one.
If your having problems you should contact support live chat. also if someone gets downed in the final area it counts as dying.
Really? First time through the map, I'm sure all 3 of my teammates went down at one point or another in the fight and I ran around the entire time, picking them up on multiple occasions (never going down myself) and upon beating it, got the task completed.Quote:

The co-op pursuits are iffy. I have run Liberate The Lost with a lot of lower EGO rating players and told them where data recorders are and what it means to survive the final fight... I've had four teams run through without anyone being downed a single time since they let me lead, even in the boss fight, and it has yet to pop up. Like I said, it's glitched and you tend to have to run it multiple times.

I completed that without getting downed and none of my team mates died (some did get downed and were revived). I did not get the checkbox. I submitted a bug report from within the game and moved on. Hopefully they will fix it eventually or I might get it on next time I play that map for more "reputation" points.
I have the same issue with Soleptor Excavation where you gotta beat the Hulker in under 3mins and I just did it again under 1min and still it won't trigger for me.
