I've had the game for 2 weeks and I still have not broken myself of this habit. Damn you WoW for giving me a false sense of awesome!

Yep, mostly blastpods here. Usually when I'm distracted taking out archers and warriors and don't realize that 6-8 pods have spawned and insta-launched with me as their aggro target. Happens less now than it did before I learned to keep my head on a swivel more.
After that, probably my own carelessness vs Dark Matter, which I made a habit of underestimating for a while. Then surprise spawn scale-ups due to not realizing other players have shown up/were out of range AFK near the area I was working. Usually tankers or blacklungs in that case.
I don't pvp and haven't really been hit by the reloading bug, so I can't blame any on those. And most of the above is happening a lot less now, whereas my deaths to the flying Hellion have increased slightly, due to the fact I seem to be holding a lot more aggro off him with my current arkfall approach.

In the short time i fought them the rogue e-rep soldiers were killing me left and right
Hulker farming/getting caught in a narrow apace and being freight trained 2-3 times in a row if he takes a funky angle.
The first tramplling is easy to survive unless it registers as multiple hits.
The second trampling will usually kill me without defensive perks and a strong shield. For XP farming, I'm pretty much glass.
The third trampling is just for good measure as The Hulk imprints his size 50 footprint upon my skull.
Going into my inventory to switch loadouts or responding to people. I tend not to care ...180 scrip worse case scenario. Had hopes for an open world pvp server.
Bulwarks, DM Monitors in groups of 3, and like everyone else, blastpods.
