I thought it was just Old Swords, but I'm having this problem on all missions. I've also discovered that enemies are attacking a place far away from where I am in stead of me... but still dealing me damage as if I were there. And running away doesn't seem to help.

Also, I used to recruit for the Anarcho-Syndicalist Revolutionary Union <ASRU Ark Hunters> (on 360 NA), but no one was interested, and I don't know if I'll ever bother playing again.
Same issue here. I can complete one session but after prompted to go to the next area it will not let me start it. I have to log off and then back on to get it to start up again, but then the circle starts again. Log on,off on...etc. There were about 15 of us doing the same mission and none of us could get it to activate. It was main story mission as well. Had no issues getting into the server today however. Seems it just started, i was able to compete 3-4 side quest before it started for me.
It is a synching issue, the objectives aren't working because the game doesn't realize you are there. See this topic for more info.
