It's what Trion calls 'pixel perfect shooting', so you can easily miss if you're not taking the time to line up properly.
When going for head shots on humanoid mobs, aim for the throat instead of the center of the face. You're more likely to land the shot that way, and it helps compensate for the 'drift' in the mouse/controller when you hit the fire button.
Using this method, I can still nail head shots on running mobs that are headed towards me, while backing up to keep them at range. I can also hit head shots when they're running sideways for cover, etc.., though that's a little harder since you have to learn to time those shots to make them count.
Make sure to take the sniper related perks if you have cloak, that helps a lot. (Crit, nano effects, etc..)
