It has happened to me a number of times randomly and when I do get booted I get a mission completed screen and get sent back to PVE. But I am still connected to the group and can instantly join back.

Yeah it's happen a lot to different clan members of mine on ps3 but I haven't even tried it. They do no about it and are trying to fix it.

Yeah, I've noticed that if I quit a Co-Op instance A, then join Co-Op instance B before instance A has finished, I will get kicked from B when instance A finishes and posts the results. This by the way means that if I join another Co-Op I will get kicked when B finishes. If I don't rejoin an instance and stay in open world, I still get the results, but don't get kicked from anything.
It's possible this instance/mission reset could affect the damage numbers in an arkfall. It's also possible that arkfall and shadow war results could kick you from instances (I haven't tested these so they need confirmation).
