So what platform are you playing on? What are the ports (past and current) that need to be forwarded to allow play? Is UPnP not showing any love (I know its buggy and not universally compatible)? Also, there are several tools available that will do router port config for you semi automatically. Let me know if you need any info or help.

Cool! Just wondering, what firewall software are you running.
For others having this problem (especially if your behind an average consumer router)
    TCP:
Port 80: [Web (HTTP) port] - is usually open by default as all your browser traffic runs through this so no forwarding is needed most of the time. You want this port outbound only unless your running a web server on you computer network.
Port 443: [Secure Web (HTTPS) port] - same as above.
Port 5000: [Undefined, UPnP] - used by Micro$oft for auto port forwarding and config. So make sure this one is open and you might not have to do anything else if your router supports UPnP and you have it enabled. Open inbound and outbound forwarding.
Port 6000: [Undefined] - Forward in and out. Probly used by the game as its control channel. This is where your game client and Trion's server talk to each other. Potentially very important.
    UDP:
Port 40000 - 41000: [Undefined] - Forward in and out bound. Do the whole range not each individual port. The kinds of ports are usually dynamic and opened and closed as needed by the game.

So if at all possible get UPnP working as it will make life a lot easier for you and save more headaches than it will cause overall. If anyone needs help with networking issues fell free to contact me.
