As far as I know you can't. I have a friend who named his character She because it was a girl and he was just trying it out. He ended up using her full time, so now his character is forever known as She.

Currently no, maybe if you talk to a GM they will reset your name. I played a game that had a GM that would let us do that. But I do not think it can currently be done, even if you talk to a GM.
