I had the same issue.. your IP address is being used in to different shards.. my friend healed my at one spawn point will my other body on another shard did missions... a cheat tank i call it.. so make sure you don't use a router.. it happens a lot on them.. once i got off a router issue was gone...

It's called Phasing. He's in a different 'phase' of the game then I am.
Did this to get my friend into San Fran. The best was he was in my vehicle and suddenly it exploded. Revived him up and across the bridge multiple times.

There's an invisible wall for the player in the phase with the robot.
Because they patched it. I did it on day 3. You can find other posters who did the same.

You could have also just fast traveled to San Fran, then have your friend 'go to' you from the friend menu.
LMAO I liked the end of the video when your buddy is just vaporized. Where did he end up? The video doesn't show you found him.
