How do you earn new titles? And at what level and where do legendary weapons drop from?

Titles are earned as you progress through the missions. Legendary weapons seem to be awarded when you buy the tier boxes, though that being said I've bought dozens and have yet to even see a orange weapon so far. And I'm at ego level 755.

Go to start > hold lt > Goals. You've all the stuff showed there. I'm enjoying my Psychopath title ^-^

I believe oranges are just rare, I got one when I was in the 400's
