Just because you're cross-hair is on free of the obstacle and on the target, does not mean you're gun is, that is the thing with TPS game. The camera position can make you think that, if the target is in the cross-hair you can hit it, but that may not always be the case.
It happens to me from time to time as well. And if you watch other player's behind cover shooting what they are covered behind, you can sometimes see that their actual guns are not free of the obstacle, on their screen the cross-hair was on the target but due to the camera angle they can't see the guns is not free of the cover.
