Something is definitely going wrong. I gave up after probably a dozen attempts to do Liberate the Lost yesterday. Not once did I get into a game that wasn't bugged.
It happened yesterday to me, tried probly 4 times before giving up. I went back like 30mins later and it worked!
I finally gave up hopefully they will create a patch for it.
It's worked fine for me every time I've played it :/. The only co-op map I'm having trouble with is Cronkhite Bunker

Yeah it always fails to join. I have gotten into it once or twice, but for some reason I can't complete two pursuits even though I've done everything right.

Odd, I've actually had pretty good luck with Liberate the Lost. But yeah, sometimes the coop matchmaking is horribly bugged.
