Gear grind works because it presents the illusion of getting better, but everything scales up. In any area you're in, your gear is slightly worse than the enemies, until it is slightly better, and finally much better making the enemies trivial. Since you're making most areas trivial with a gear grind, they lock you into farming the same end zone until the new expansion comes out. It works, sure, never said it didn't. If it didn't work, they wouldn't be collecting subscription fees as long as they have.
The lack of obvious/dramatic progression has a reason, and it's for accessibility. Doing it this way means anyone who decides to play one day will hopefully be able to experience the game as well as someone who's been playing for a year. I'm not sure anyone is scared of a grind, but I know a lot of people are bored of it. I certainly am. You want an XP grind, try EQ...that's some ugly grinding.
Pursuit grinding isn't needed, many of those pursuits are meant to be passively obtained, not going out of your way to get them.
