Same problem here
Sames here, ive had the problem of reloading only 4 rounds of a shotgun, or sometimes loading the full clip and then continuing back to 1 or none.
Yea I just came onto the forums to complain also about this its not only annoying but its getting me killed 9 out of 10 times where you go to reload and then once you finish your ammo is still 0 and character gose for another reload. Also to note that when you still have ammo in the clip left you reload it will finish and you will only have what you had in your clip before the reload, so the reload itself somehow is bugged.

this is the ammo bug, it happens when you reload and hit and keep getting hit. you keep loosing your amo clip for each shot you take until you are out. it has been in the game for at least 3 months now.
It also seems that you replay the reload animation if you press 'fire' too quickly during reloading. Very annoying.
'Double Reload' is most often caused by the following scenario:
If you have a perk that allows for faster reload under certian conditions, and those conditions cease to be true before the completion of the reload action, then it invalidates the reload and makes you start over.
This is most obvious when used with Overcharge and the 30% faster reload perk with Light Machine guns.
Because of this, all of the perks that increase reload speed situationally (faster reload when shields down) interrupt and reset your reload entirely if the conditions granting you that reload speed go away (someone heals you at ALL).
There may be other scenarios where this happens, but I never had this happen before I was attempting to increase my reload speed.
Edit:
Activating your ego power does not reload your weapon unless it is Overcharge, which reloads your weapon when activated.
Oh I guess that explains it, I use over charge power.
