The best MMO ever was the first really major one, Ultima Online (UO). Yes, I know there were problems, but it had sooo many features that it was the first to incorporate. Most of those features went on to become MMO standards. Things such as crafting, banks, housing, guilds, mounts, pets, open world sandbox, loot drops, treasure chests, PVP, and countless other features Most games since have simply built on the ideas that UO started and added one or two new twists. Thats why so many MMORPG all "feel" the same after a while. It is because they all have the same grandpa, thats why they seem so similar.
Heck, UO had more gameplay features way back then than Defiance does now. Not hatin', just sayin'.

Planetside. Pre-Core Combat, before all the BFR powshtako caused everyone(including me) to leave. Most fun I have ever had in a MMO. ever. I meant that literally. it was amazing.
second favorite is DAoC.
3rd favorite was Vanilla WoW.
I would say I am having a hard time deciding if my all time favorite MMO was either Guild Wars or Guild Wars 2, cause I couldn't pick which one I like the most, but seeing how Guild Wars 1 is in fact not even an MMO, that means Guild Wars 2 is my all time favorite MMO.
LOTRO is still my current favorite. Before that it was DAoC. Before that (in the dark ages of online gaming) it was The Realm Online! -)

Tradewars 2000and somethingish ! Not a true mmo but was fun back in the day. First online multiplayer game I played. Good old BBS days! Lol
PSU and the PSO games aren't really MMORPG's.
My favorite is a toss up between SWG Pre-CU and MxO.
Other than Defiance, I really liked Asheron's Call 2
It had some very cool classes
