I've had the issue now for a few days (ever since I purchased the perk) where the preparedness perk doesn't equip. The issue specifically is, it is showing in my selected perk list, but the effects of it aren't working. When you go to the EGO grid it says it is unequipped even though it is. To resolve the issue I usually have to equip another perk (Any) and then re-equip preparedness after that for the [E] symbol to come over it (equipped).
I haven't had that issue on my Preparedness class, however the perk for extending ego time with kills is not working for me even though it's equipped.
Maybe it's linked to what perks you have equipped. Does the perk that doesn't work for you show up in your perk grid but says it's not equipped in your EGO grid?
No, I've equipped/unequipped multiple times. Mainly just used it for Arkfalls and such and realized it wasn't extending any time as I killed hellbugs.
I have this problem too. The first time it disappeared from my equipped list. But mostly it sits there not functioning.. I have to equip another, and then equip preparedness again. Just like OP. This happens on loadout swaps and on relogs.
This is still happening to me. I've reported in in-game too. Quite annoying to have to frantically rush to swap it out and back in before a co op or pvp match!

Let me guess, it's the last perk that you've unlocked and it just randomly unequips itself. I really wish that they'd fix this f'n bug. It's almost as game breaking as the reloading bug.
