How do you unlock all 4, do you have to have a direct link from your first one to the second & the 3rd etc.

No, but you need to have enough EGO Points to fill your perks in order to "link" your 4 EGO Powers.
Just draw a "lines" between these EGO Powers picking the most useful ones at the beginning and then you can chose individual EGO Powers for different loadouts.
Yup, I've all 4 EGO powers maxed out. Had two around EGO 200 because of the free scavenger perk. Linky linky.
