Logout & log back in, and see if it fixes it, the code count being off is a common issue...
Done that a few times, I know it's a known issue but that usually seems to be the case for the numbers being +1. I don't really care though, oh well.
If you've used them all and they've all been refused when entering them a second time.. Well it sounds impossible to always miss the same code in such a big list. Just give it time, if it doesn't fix itself you can always pm the CM's for help.
